Improving the handover experience
Aim:
The project aimed to improve the quality, consistency and flow of communication during handover.
Who was involved:
Residents, relatives & staff.
My Home Life Tools and Resources used:
My Home Life Frameworks, Image cards and Emotional touchpoints.
Quality Improvement Project:
Participants shared their particular homes handover and collaborated together to develop a handover sheet that would improvement consistent and high quality communication between staff members working in a care home setting. This handover sheet could be used and adapted by others.
Key learning:
· Collaboration brings people together to share skills, solve problems faster, and spark new ideas.
· Improving the handover experience helps to reduce errors, save time, and build trust. Ensuring critical information is shared clearly, reduces miscommunication between shifts or teams, and creates a smoother, safer workflow for everyone involved.
